$125 - 1.5 hr Evaluation and Treatment

$100 - 1 hr return visits

Insurance - We are a provider for Occupational Therapy services:

• Workman’s Compensation injuries (with pre-authorization)

• Motor Vehicle accident reimbursement (with pre-authorization)

We accept: Cash, check, Visa, Mastercard, Discover, American Express, Flexible Spending Account and Health Savings Accounts

NOTE: We do not file for “out-of-network” insurance, however, we will gladly provide you with a superbill to use for your own record.

For Workman’s Comp and MVA insurance billing, we require a written prescription from your physician that states “Occupational Therapy (OT) to evaluate and treat as indicated”. A medical diagnosis (ICD code) is necessary to be able to file through insurance. Click below on the Dr. Referral Form that can be printed and given to your physician.
